Certification and Recertification
Pregnancy Certification
When: Anytime during pregnancy, ideally during your first trimester
What we need:
-
a picture of your ID
-
proof of your current address (a piece of mail or your ID)
-
proof of household income (e.g - pay stubs, unemployment benefits, child support, Social Security benefits)
-
proof of your pregnancy (an ultrasound or letter from your doctor with your due date)
-
hemoglobin (iron) level

You will automatically qualify for WIC if you are enrolled in Husky A, C, or D, or if you have SNAP benefits or cash assistance.
Infant Certification
When: Call us when your baby is born. We certify babies as soon as possible so we can give you the infant feeding support you need.
What we need:
-
a picture of your ID
-
proof of your current address (a piece of mail or your ID)
-
proof of household income (e.g - pay stubs, unemployment benefits, child support, Social Security benefits)
-
your baby's weight and length from birth (on the "crib card" or discharge papers)
*if you are certifying your baby when they are older than 2 months old, we will also need a height and weight from within 60 days of your WIC appointment

Your baby will automatically qualify for WIC if they are enrolled in Husky A, C, or D, or if you have SNAP benefits or cash assistance.
Postpartum Recertification
When: Typically 4-6 weeks after your baby is born, or the appointment following your baby's certification
What we need:

a picture of your ID
-
proof of your current address (a piece of mail or your ID)
-
proof of household income (e.g - pay stubs, unemployment benefits, child support, Social Security benefits)
-
your postpartum hemoglobin (iron) level
You will automatically qualify for WIC if you are enrolled in Husky A, C, or D, or if you have SNAP benefits or cash assistance.
Child Certification (and Recertification)
When: Yearly from the time you certify, up to the 5th birthday
What we need:
-
a picture of your ID
-
proof of your current address (a piece of mail or your ID)

proof of household income (e.g - pay stubs, unemployment benefits, child support, Social Security benefits)
-
your child's most recent height, weight, and hemoglobin (within the 60 days prior to your WIC appointment)

Your child will automatically qualify for WIC if they are enrolled in Husky A, C, or D, or if you have SNAP benefits or cash assistance.
Mid-Certifications
We conduct "mid-certifications" every 6 months. These are more in-depth visits, which review your child's growth chart. For children under 2 years old, the doctor can send us a copy of their chart with their height and weight. For older children who do not have a doctor's appointment, you will bring your child to the WIC office for measurements.
6-month Infant Mid-certification
When: soon after your baby's 6 month visit with the doctor
What we need:

your baby's updated height and weight from their 6 month doctor's visit

*Please do not rely on the doctor to send us this information. Obtain it yourself and email it to meridenwic@meridenct.gov

18-month Child Mid-certification
When: Soon after your child's 18 month visit with the doctor
What we need:
-
your child's updated height and weight from their 18 month doctor's visit
-
your child's updated hemoglobin from their 18 month doctor's visit
​
*Many doctors do not draw blood at 18 months unless you ask for it. You may need to remind them that WIC requires a hemoglobin (iron) check to be done at 18 months.

Older Child Mid-certification
When: half-way between recertification appointments, Typically at 2.5, 3.5, and 4.5 years of age
What we need:
-
your child's updated height and weight
*If you have this information from a recent doctor's visit (within 60 days of your WIC appointment), please obtain the printed information from your child's doctor and email it to our office (meridenwic@meridenct.gov)
*If you do not have this information from a doctor's visit, you will need to bring your child to the WIC office to be weighed and measured.
Postpartum Mid-certification
When: If you are still breastfeeding (even once a week counts!), you will be scheduled for a mid-certification.
What we need:
-
your updated weight from a recent doctor's visit (within 60 days of the WIC appointment), or an in-person visit at the WIC office to obtain your weight

If you are breastfeeding a little bit, your food package will stop at 6 months.
If you are primarily or only breastfeeding, your food package will keep going up until 1 year at most.
Follow-Ups
Follow-up appointments are conducted every 1-3 months, depending on what we need from you. This is typically a quicker appointment, and may be a simple check in with a nutritionist to touch base on your health goals. It may also be an opportunity for us to collect information from you that we may be missing.
Information we may need during a follow-up visit:

a new prescription form to continue providing your baby's special formula

bloodwork or height and weight information missing from a previous appointment
